Attached is the:
1. Terms and conditions to be a HSP panel clinic (for info)
2. Tentative briefing schedule for interested doctors. All places and date where briefing are to be held are confirmed.  It is at at 2.00pm on the mentioned date,   however the venue is not finalised as yet. (I will keep you posted)
3. Attendance form to briefing “BORANG PENGESAHAN KEHADIRAN”.
